What's on a standard field trip permission slip?

Trip details (destination, date, departure + return time, transportation, cost), student identification (name, DOB, classroom), parent contact for the day (mobile phone), medical info (allergies, conditions, insurance, pediatrician), and a parent signature authorizing the school to obtain emergency medical care if needed. Most schools use the same template year after year — only the trip details change.

Why do I have to fill out my kid's medical info AGAIN for every field trip?

Liability. The classroom teacher leading the trip needs the medical reference physically with them, not in a folder back at school. Same data + same insurance card every time, but it has to be on the slip going on the bus.
